Root Cause Analysis can help auditors write better audit recommendations, thereby increasing audit impact. This virtual course introduces hands-on tools, that, when thoughtfully applied within each phase of the audit process, enhance audit quality and allow auditors to make recommendations that address the underlying causes of observed deficiencies.

Participant Profile

This course is for auditors who want to understand and use root cause analysis techniques to identify the issues that matter and write targeted recommendations that will result in effective solutions for the auditees.

Course Structure

The virtual course is divided into 4 sessions, each lasting 3.5 hours.

Participants will connect online through the course platform, Adobe Connect. They will view the instructors as well as the presentation live on screen and have access to the chat and question features. The course includes a mix of instructor-led content, interactive exercises and break-out sessions.

  • Root Cause Analysis and the Audit Process

Day Two

  • Five Why’s Technique
  • Fishbone Diagram Technique
  • Cause Mapping
  • Pareto Charts

Day Three

  • Main Categories of Possible Root Causes
  • Governance-related Categories
  • Operations-related Categories
  • Culture as a Root Cause

Day Four

  • Cognitive Bias
  • Root Cause and Audit Recommendations
  • Course Review

Course hours can be submitted for CPE or CPD credits, subject to approval by IIA or CPA Canada.
